Vasectomy Reversal

vasovasostomy /væsouvæˈsɑstəmi/

the reversal of a vasectomy, performed by surgical reconnection of the severed ends of the vas deferens

Mini-Incision, No-Scalpel Vasectomy Reversal

What Is a Vasectomy Reversal?

A vasectomy reversal restores the natural flow of sperm by reconnecting the vas deferens, the tube that transports sperm from the testes to the urethra. This procedure is ideal for men who wish to regain fertility after a vasectomy.

Local Anesthesia for Comfort and Flexibility

We perform the procedure while the patient is awake, using local anesthesia with or without mild sedation. This approach ensures comfort but also allows us to evaluate the testicular end of the vas for the presence of sperm during the procedure. If no sperm is detected, we can have an informed discussion with the patient about whether to:

  • Proceed with the reversal
  • Switch to an epididymovasostomy (a more complex procedure)
  • Abort the operation

Performing the procedure under local anesthesia keeps costs significantly lower than general anesthesia, though the latter option is available if preferred.

Mini-Incision Technique

Access to the vas deferens is achieved using a refined method similar to the no-scalpel vasectomy approach. This minimally invasive technique reduces tissue trauma and the risk of bleeding.

Microsurgical Precision

Microsurgical vasectomy reversal has largely replaced older techniques, such as repairs with the aid of stenting or loupe magnification. The operating microscope provides enhanced visualization with 5x–40x magnification. This is ideal for re-approximating the vas deferens, a delicate structure with a 2.0 mm external and 0.5 mm internal diameter.

Success Rates and Factors

After a vasectomy reversal, it usually takes around three months for sperm to reappear in the ejaculate, but in some cases, this process can take anywhere from six to 12 months or more. The most important factor in predicting the success of a vasectomy reversal is the time elapsed since the vasectomy. According to a large multicenter study, the patency* and pregnancy** rates for men who underwent vasectomy reversal were:

  • Less than 3 years: 97% patency, 76% pregnancy rate
  • 3–8 years: 88% patency, 53% pregnancy rate
  • 9–14 years: 79% patency, 44% pregnancy rate
  • More than 15 years: 71% patency, 31% pregnancy rate

* Patency Rate: This refers to the percentage of cases in which the vas deferens is successfully reopened after a vasectomy reversal, allowing sperm to flow through it. It is confirmed by the presence of sperm in the ejaculate during follow-up semen analysis.

** Pregnancy Rate: This measures the percentage of cases where a successful pregnancy occurs following the vasectomy reversal. This rate reflects the combined effectiveness of the procedure and the couple’s overall reproductive health.
